Media Consent Form - Child/Student

Complete this form on the Audiri App.  Permission is needed for your child/ren to have work and/or photos published on our new official Facebook page (NB  names will not be published)

NB:  This is not the same form as the usual Permissions & Consent form that needs to be updated each year.  It is a new, once off, consent that will hold for the time your child/ren attend this school.

Family Information Update

Please complete the 'Change of Details' form on the Audiri App to update any of the following changes to your circumstances:

*  home address (residential and/or postal)

*  contact phone number

* employment &/or place of employment

* email address

* emergency contacts

* student's medical condition

* *shared living arrangements

** custody information

Healthcare Plans & Medication For Students

Parents & Caregivers, are you up to date with your child's healthcare plan & medication?

If you are unsure please contact the Front Office to check and act on reminders or requests sent to you from us regarding  your child's healthcare needs.

We wish to ensure that your child is supported appropriately for their medical condition during their time at school and for activities such as swimming lessons and camp.

It is recommended that healthcare plans and medication agreement forms be reviewed every 12 months.

Parent Volunteers Transporting Students

We are very appreciative of our parent volunteers at this school especially when it comes to transporting students to sporting events or special activities. 

We aim to keep our students safe at all times including when being transported privately which involves extra checks and balances.

Volunteers transporting students need to follow the following process which aligns with department requirements:

*  have a current Working with Children Check (valid 5 years) 

*  use a roadworthy, registered vehicle

*  have a current driver's license

*  show car registration details and drivers licence to staff for checking validity, on the day of the event, prior to leaving time

*  all of the above applies each time you transport children.

Please note that apart from the working with children check, the school does not store car registration & drivers licence details as these can change throughout the year.
